Its important to keep your medical information up-to-date and in one place in the event of an emergency. Take responsibility and prepare for medical emergencies and natural disasters.
This Personal Medication Record should be printed out and completed and kept in a safe place. A few choices are secured on your refrigerator or in your glove compartment. Or copy one to put in each location. Included is a wallet card for the basics. Cut it out and place in your wallet.
This tool helps to ensure effective and correct medical treatment by emergency responders in the event of a medical emergency.
Consider adding the following information for it to be especially helpful:
A recent photograph
Advance Directives
Date of last update
A local and out-of-state emergency contact
Your social security number
Location where medications can be found in your home
A separate form for each member of your household
Keep the information as up-to-date as possible by coming back to this website to print out a new form when changes occur. We will also be adding a form that can be saved on your computer and edited in the near future.
